- Booking information. Appointments booked through our online scheduler are handled by our scheduling provider, which collects the details needed to create your appointment.
- Information collected automatically. Like most websites, we collect standard technical data such as IP address, browser and device type, pages visited and referring site, through cookies and similar technologies.
How we use it
We use the information to respond to your inquiry, schedule and confirm appointments, verify insurance benefits when you ask us to, improve the website, and meet our legal and professional obligations. We do not sell your personal information.
Health information and HIPAA
Please do not send detailed health information through this website. Website forms are for scheduling and general inquiries and are not a secure medical record. Protected health information created in the course of your care as a patient is governed by our Notice of Privacy Practices under HIPAA, which is separate from this policy and is available at the office on request.
Analytics and advertising
We use Google Analytics and Google Tag Manager to understand how the site is used, and we may run paid search advertising. These tools set cookies and may record aggregated, non-medical usage data.
